Job Description:

The Registered Nurse Clinical Coordinator (RN CC) supports Clinical Team Managers in planning and implementing quality care by advancing key workflows throughout the patient journey. This position reviews all referrals for appropriateness to Behavioral Health home care, monitors the accuracy and documentation of Homecare Homebase (HCHB) inputs and follows-up to ensure workflow requirements are satisfied. The RN CC also acts as a key intermediary between the patient, the CTM, the patient’s physician or medical provider, and other facility staff overseeing patient care as well as a back up to the CTMs and Clinical Navigators or supervising field staff including home health aides as needed.

As an Elara Caring Registered Nurse Clinical Coordinator, you’ll contribute to our success in the following ways:

  • Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Behavioral Health team.

  • Reviews referrals for appropriateness to be serviced under the Behavioral Health service line.

  • Review medication lists and reconcile medications with providers.

  • Reviews and clears HCHB coordination notes and processes administrative task workflows in an accurate and timely manner.

  • Obtains orders from physicians, including verbal, written and faxed orders, and enters into appropriate system for CTM review and approval.

  • Identifies areas of concern related to physician orders or other clinical documentation and escalates to CTM for resolution.

  • Reviews, edits, and approves Face-to-Face (F2F) encounter and follow-up on unresolved F2F encounters scheduled post-SOC. Communicates with RN when F2F is obtained after 485 has been processed to ensure accurate diagnosis consistent with plan of care. Resolves issues by engaging CTM or MD as required.

  • Monitors patient hospitalization list, confirms status, and coordinates appropriate resumption of care.

  • Discharges patients in inpatient facility at end of episode (EOE).

  • Obtains patient-related attachment renewal.

  • Serves as a back up to the Behavioral Health CTMs and Clinical Navigators reviewing SOC/ROC/Recerts in their absence.

  • Performs timely supervisions of field staff and HHA's as necessary.

  • Oversees uploading and tracking of medical information by utilizing various EHR (Electronic Health Record) systems.

  • Reviews, updates, and maintains the At-Risk registry.

  • Participates in High-Risk Meetings and ensures that the best provision of services is offered.

  • Adheres to all applicable county, state, and other regulations.

  • Available to provide direct care, as needed.

  • Answers telephone calls from patients, patient representatives, and medical professionals and communicates messages to the appropriate party or parties.

  • Provides clerical support to patients such as but not limited to scheduling follow-up appointments for patients (as requested), providing health summaries to case managers, obtaining pre-authorizations, and addressing patient questions or concerns.

  • Prints patient instructions and sends to patient.

  • Performs chart audits as required by Elara Caring policy.

  • Reviews print signature forms (consent, Advanced Beneficiary Notice (ABN), F2F), including the scanning, attachment, and entering Forcura or appropriate system.

  • Monitors and resolves fax status reports and incoming faxes from Forcura; ensures all communications are received. Re-faxes as needed.

  • Faxes episode summary reports to facilities (e.g., hospital), as required.

  • Verifies receipt of notice of Medicare provider non-coverage to patient.

  • Supports key patient initiatives to ensure successful implementation and maintenance (e.g., pandemic response, flu shots, etc.).

  • Ensures office coverage in the absence of a Branch Director, Office Manager or CTM.

  • Manages medical inventory, including supply ordering and shipment/distribution to patients or employees.

  • Promotes Elara Caring’s philosophy, mission statement and administrative policies to ensure quality of care.

  • Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.

  • Performs other duties/projects as assigned.

What is Required?

  • Associate degree in a Nursing related field required

  • 2 years of experience as a registered nurse in an appropriate clinical care setting

  • Home Health or Hospice experience preferred

  • Current Licensed Registered Nurse

  • Excellent communication, time management, technology, and conflict resolution skills

  • Valid driver’s license and insurance

  • Reliable transportation to perform job tasks
